  • NDIS Worker Screening Check or willingness to obtain (required) *see below
  • Current First Aid Certificate or willingness to obtain (required)
  • Two work referees (required)
  • Certificate III or IV in Disability, Aged Care or related field (desirable)
  • Proven experience record (desirable)
  • Current driver’s licence and a reliable, insured vehicle (desirable)

Applying for the NDIS Worker Screening Check

  • For full information, go to https://www.vic.gov.au/ndis-worker-screening-check
  • Please try to apply online first – you will need three different identity documents and a smartphone with camera.
  • If you apply manually, please take a photo of your signed application forms and the addressed, dated express envelope before you post your application, as proof of your application.
  • It presently costs $123.20 which is tax deductable.
  • We cannot accept a Working With Children Check (WWCC) instead of the NDIS Check. If you have already paid for a WWCC you can request a refund via the NDIS Worker Screening process.
  • We can only provide you with our NDIS Registration number to apply for the NDIS Check if you have proven a suitable candidate for a position with our company with the correct ID, paperwork, interview and references.
  • If you have applied for your NDIS Check and are waiting for clearance, we can accept you to work with our clients (under supervision by another Supreme Health worker) if you can provide proof of applying for the check (an email from the NDIS Worker Screening Team, or a photo of your manual application as above) PLUS a clear Police Check AND a signed & witnessed Statutory Declaration.
